To comply with State regulations, all adults who participate in youth activities are required to have a current
WATCH form on file. Please download, complete and submit one to the
church office if you have not done so within the last year. Holy Spirit needs to have a check run by the Department of Motor
Vehicles (DMV) on any person who will drive our vans. This is done in order to maintain our insurance coverage. If you plan to drive
the church van for any event, please fill in a DMV Check Form and submit it to the church office.
The use of all facilities at HSLC is scheduled by the Office Manager, Mary Chamberlin. To request the use
of HSLC facilities, please fill out the form and turn in to Mary.
